New Patient Registration (updated Oct 23)

If you live within the Practice area and wish to register as a patient, please bring your NHS Medical Card containing your NHS number with you. One of our receptionists will give you a registration pack for completion and return to the surgery.  Alternatively you can download one (see below) and print and complete both forms at home, and then bring them along to the Surgery.

Once completed, you will be asked to make an appointment for a new patient health check with one of our nurses. Please bring a urine sample for that appointment. 

Also please see the downloadable link for the Proxy form for those patients who wish to give formal proxy for other family/carers etc to discuss and deal with their healthcare needs and requirements - this should be completed as per the instructions on the form.

Access to the Surgery

Free 2-hour parking is available in Bell Mead. Two parking spaces for the disabled are located behind the Surgery. The surgery has wheelchair access and the patients' toilet has facilities for the disabled.

An induction loop system is installed at reception for patients wearing hearing aids.

Please ask for our portable unit when you see a doctor or nurse.
