New Patient Registration

If you live within the Practice area and wish to register as a patient, please bring your NHS Medical Card with you. One of our receptionists will give you a registration pack for completion and return to the surgery. Once completed, you will be asked to make an appointment for a new patient health check with one of our nurses. Please bring a urine sample for that appointment.

Access to the Surgery

Free 2-hour parking is available in Bell Mead. Two parking spaces for the disabled are located behind the Surgery. The surgery has wheelchair access and the patients' toilet has facilities for the disabled.

An induction loop system is installed at reception for patients wearing hearing aids.

Please ask for our portable unit when you see a doctor or nurse.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
