Accurx Online Digital Request Forms for all patient access requests
All patients should now use the new "Accurx" digital request form, located at the top of the main home page.
It is a simple to use online request form/tool that when sent arrives in our email box and is then taken for Triage. All clinical requests will be seen by the duty GP and all admin work will go to the admin office with responses to all patients within 24 - 48 working hours.
Early feedback is very positive as it takes away the need to call or visit the surgery, unless you dont have access to IT or a smartphone etc - if you dont have access to a Smartphone or laptop, our staff are ready and happy to assist any patient.
Cancelling or changing an appointment
If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.
To cancel your appointment:
- use your NHS account (through the NHS website or NHS App)
- using the GP online system – SystmOnline
- phone us on 01277 352 224, Monday to Friday from 8am
If you need help when we are closed
If you need medical help now, use NHS 111 online or Call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
When we dont have any appointments left on the day all patients have access to both our local regional Urgent care centres based at Harold Wood and Orsett Hospital
If you need help with your appointment
Please tell us on your Accurx request form:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if when you visit for your appointment you need a chaperone
- if you need an interpreter
- if you have any other access or communication needs
Home visits
For urgent home visit requets please call the Surgery before 1030.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls
You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Extended access
Brentwood and Ingatestone's Extended Hours GP Hub (ACE Limited)
We are a member practice of ACE (Accountable Care Enterprise) Limited, the GP Hub we share with the 7 other Brentwood PCN practices, and which is based at the Brambles Surgery, Greenwich Avenue, Brentwood, CM15 9DY
The link leaflet below gives full details on the support and out of hours services they offer our patients and how you can access them. ACE also run the various housebound vaccination programmes and we ask that all housebound patients ensure they check/confirm they are on our housebound list at your next appointment or telephone call with us.
Reminder to all patients: ACE Hub do not administer general vaccinations, these must be done at our Surgery.
