New Patient "Online Registrations" Information (updated May 24)

Dear prospective new patients, we now offer online registration for new patients. Please use the URL link below which will take you to the GP Registration Service but also please check that your address is within out catchment area (see the 2nd link below).  As soon as the new online registration is complete, we will then receive a notification and then add in the relevant information to our patient records system for the new patient.

We strongly encourage patients to register via this online service to reduce the need for the multiple page printed documents and to speed things up for the overall process.

The link below shows the NHS England approved practice catchment area - when open, please type in the postcode CM4 0FA for the Surgery and then narrow down/zoom into the actual line bordered catchment zone.  This covers just up to but not in Blackmore Village to the North West, to Maldon Road Margaretting to the North East, down to Old Church Lane/Mountnessing Road in the South and to Roman Road by A12 in the west.

For those new patients unable to access or use online services

If you live within this boundary catchment area and wish to register as a patient, please bring your NHS Medical Card containing your NHS number with you. One of our receptionists will give you a registration pack for completion and return to the surgery.  Alternatively you can download one (see below) and print and complete both forms at home, and then bring them along to the Surgery.

Once completed, you will be asked to make an appointment for a new patient health check with one of our nurses. Please bring a urine sample for that appointment. 

Also please see the downloadable link for the Proxy form for those patients who wish to give formal proxy for other family/carers etc to discuss and deal with their healthcare needs and requirements - this should be completed as per the instructions on the form.

Access to the Surgery and Parking

Free local authority 2-hour parking is available in Bell Mead via the public parking bays on Bell Mead road and in the small car park opposite the staff allocated bays.

Two parking spaces are allocated for those patients with access needs, although on some very busy days our clinical staff use them.  If you need an access parking bay please let the staff know at time of making your appointment booking or when at the Reception desk and we can note the time/day for this purpose; our staff will move any vehicles to allow patient access. 

The surgery has wheelchair access and the patients' toilet has facilities for the disabled.

An induction loop system is installed at reception for patients wearing hearing aids.
